Juliet:"What's in a name? That which we call a roseby any other name would smell as sweet." Okay, show of hands. Who would prefer we change the name of our profession? Should it be Physician Assistant or Physician Associate? Or, something different altogether? Physician Assistant Patients arealreadyconfused by what we do. Wouldn't a name change just confuse them even more? Part of this patient confusion comes from the term assistant. It assumes (rightfully so) that we are indeed "assisting" someone; in this case a doctor. The term itself would lead one to believe that at some point during their office visit they should have time with the actual physician for whom the PA is assisting. As a family practice Physician Assistant in a rural setting, I usually work alongside other PAs and Nurse Practitioners. Nowadays rarely is there a doctor to be found on site. And that is OK; there is always someone available by phone if necessary. In other words, I rarely "assist." But, this, in my opinion, is just semantics. I am proud of what I do as a Physician Assistant and the high quality of service I bring to my patients. When my patients ask me when I am going to become a doctor, I just smile and explain the millions of reasons why I love my job, why I became a PA and why even if given a free "upgrade" to MD I would never even consider it. Physician Associate The term associate is not absent of ambiguity. Just look at one of the definitions assigned by Wikipedia: Associate, a person who is in league with theMafiabut is not treated as afull member, e.g. a corrupt official. Not that there isn't a bit of a "cool" factor that comes with being likened to the mafia. I think the big complaint about the term "assistant" is that it comes with a feeling of subservience. In a law firm, an associate is a low-level lawyer. The problem with this is that PAs are not low-leveldoctors they are something entirely different. Maybe the main problem is the term Physician A physician is at the highest level of the medical field. And therefore, the term should be reserved for just that. For a comparison take a look at variations of the term Nurse: Nurse Nurse Practitioner Certified Nurses Assistant Licensed Vocational Nurse Nurse Midwife Nurse Anesthesiologist This professional deviation from the term "nurse" is far removed from the original intention. Outside of the medical field are other types of professions that exist alongside another "main" occupation; Paralegal comes to mind. One could make the case then for the term Para-medical instead of Physician Assistant, but this is just too confusing. Especially given the closely related term paramedic. A New Profession: In all honesty, the physician assistant of 2018 is a lot different from the PA of 1977. Part of what has happened is that we are coming to terms with the fact that to be an excellent diagnostician in primary care one does not need to attend medical school and complete a residency. There is a minimum effective dose for primary care, and that is probably the education of a Physician Assistant. Of course, while working alongside a physician in a surgical rolethe term assistant is always appropriate. So maybe what we need then is an entirely new professional designation. It is not the "Assistant" or "Associate" designation we should be concerned about but the term "Physician"! Otherwise we will always be playing the same game, creating the same patient confusion and constantly struggling to find our own identity. After a flight home from a business trip, Jack finds his apartment destroyed by an explosion. He calls Tyler Durden, a soap salesman whom he befriended on the flight, and they meet at a bar. A conversation about consumerism leads to Tyler inviting Jack to stay at his place; outside the bar he requests that Jack should hit him. The two engage in a fistfight, and Jack subsequently moves into Tylers they have further fights outside the bar, and these attract a crowd dilapidated house. of men. As Jack and Tyler begin to use physical violence as a means for release and an opportunity for the expression of the male ego, Jack begins to come to a deeper understanding of his own capabilities. This disorder is characterized by disruption of one’s identity coupled with travel away from home. While Jack’s consciousness is at rest, the personality, Tyler, becomes active, free to move about the country and pursue his plan. His plan of Operating Mayhem, leaving Jack very confused to wander the path of Tyler, unraveling the plans suffering from massive bouts of Deja vu. The primary mental disorder portrayed in the film, contrarily, is not Insomnia or Dissociative Fugue, but rather Dissociative Identity Disorder. The main character Jack as I thought he was suffering from insomnia in the beginning of the movie, really has a disorder called Dissociative Identity Disorder (DID). Dissociative Identity Disorder involves extensive memory disruptions for personal information along with the presence of two or more distinct identities, or personalities within a single person. For example, each personality has its own name and is experienced as if it has its own personal history and self-images. These different personalities can also be known as alters or alter egos. As far as the protagonist Jack his alter ego is Tyler. The split between the relatively passive Narrator (Jack) and the quick-witted flamboyant Tyler Durden is characteristic of such a split. Usually the core personality is passive, dependent, guilty, and depressed but the alternates may be hostile, controlling and self-destructive. Tyler exploits on the arena-floor of Fight Club, as well as his propensity for assuming command mark him as noticeably removed from the primary personality, which is Jack. As the film draws towards the climax, Jack and Tyler struggle for supremacy. Jack is aware now that Tyler orchestrated the events which led to their meeting, and possessed of a need to stop Tyler from seeing Operation Mayhem through its final success in the destruction of several credit card headquarters buildings. Jack finally catches up with his alter ego Tyler; he is faced with the task of stopping what is essentially his better half. In a moment of defeat, the Narrator finds himself at the mercy of his split personality. Jack is being held at gunpoint by his alter-ego and forced to watch the destruction orchestrated by Tyler. Odyssey When one ponders the Greek mythology and literature, powerful images invariably come to mind. One relives the heroes' struggles against innumerable odds, their battles against magical monsters, and the gods' periodic intervention in mortal affairs. Yet, a common and often essential portion of a heroic epic is the hero's consultation with an oracle or divinity. This prophecy is usually critical to the plot line, and also to the well being of the main characters. Could Priam have survived in the Achaean camp if not at the gods' instruction (200-201)? Could the Argos have run the gauntlet of the Prowling Rocks if not for the gods' advice of using a sacrificial bird (349). Moreover, prophecy can be negative as well as positive. Achilles was prophesied to die gloriously in battle if he chose his life's way as a warrior. Oedipus was exiled and condemned by his own words, after he slew his sire and wed his mother. This type of prophesy can blind even the gods themselves; Chronos was fated to be defeated and his throne stolen by his son. Demeter loses Persephone periodically every year because her daughter ate Hades' pomegranates. Prophecy plays an important role in the whole of Greek folklore. Something this ever-present bears further examination. In The Odyssey, prophecy in its myriad forms affects nearly every aspect of the epic. Prophecies are seen in the forms of omens, signs, strict prediction of the future, divine condemnation, and divine instruction. Though conceptually these forms are hard to distinguish, they are clearly separate in the Odyssey. Moreover, prophecies can be interpreted not only on the "plot device" level, but also on the level of characterization. Whether a character accepts or denies the gods' prophecies tells the reader something about the character himself. Omens are brief prophecies intimately connected to the action at hand, which must be interpreted in terms of that action. Halitherses comments on the eagle attack after Telemakhos condemns the suitors (463-464); he correctly interests it to mean that if the suitors keep feeding off Odysseus's possessions they will be destroyed. Yet the suitors ignore the omen, inviting their eventual destruction. This haughty treatment of a divine omen is a justification for their deaths. When Penelope says if Odysseus had returned he would, with his son, surely slay the suitors, Telemakhos let loose a great sneeze (429). This omen reinforces the previous one, and simultaneously prepares the reader for the carnage to follow. However, not all omens are effective. In the case of Telemakhos we see many bird omens signaling for him to do something about the suitors. Whether it was his immaturity to interpret the bird omens or blind arrogance Telemakhos does not act on them. In fact, it's not until Athena comes to him that he thinks to take action against the suitors in his house. Signs are similar to omens, but differ in one crucial aspect; the prophesee is looking for a specific omen in order to decide whether he should or should not take some action. There is only one good example of a sign in the Odyssey; on page 460, Odysseus asks Zeus for two divine signs to decide if it is time to slay the suitors. Zeus answers with a thunderclap from a cloudless sky and allows Odysseus to overhear a maid's prayer for vengeance. Because of these signs, Odysseus begins his plan to slay the suitors. Later on, with a thunderclap Zeus actually signals for the precise time to strike. Signs are helpful devices; they allow not only a rationalization for when an event occurs but also shows the approval of the gods on such an action. Not only are signs and omens plentiful in the Odyssey, but also the type one usually associates with prophesying, strict prediction of the future, abounds as well. Penelope states that she will marry the man who can string Odysseus's bow and perform his famous feat (469). Since Odysseus is the only one to do so, the prophecy is fulfilled. This "prophesy" is just a statement of the future; it contains no judgmental quality whatsoever. Theoklymenos's prophesies to Penelope that Odysseus is at hand on the island and plotting vengeance on the suitors (417) This, of course, is already true, so the prophecy is technically true as well. However, it makes no judgement on the rightness or wrongness of either Odysseus's or the suitors' position.
